Registration 2010-2011 High School Sports
Step #1:
The high school sports registration forms are available under the "Registration Forms" link to the left. To obtain the form, please click the link to the left or CLICK HERE to proceed to that page now. Please fill out the form and return it to the VMA Athletic Office by the due date with your payment.
Step #2:
For KHSAA Physical Forms, please visit the the "Registration Forms" link to the left or CLICK HERE to obtain the form directly from the KHSAA website. Print and Return all pages to the Athletic Office along with the registration form. These forms are also available in the athletic office.
Step #3:
Villa High School Sports Policies. All students and parents must read and initial each policy and submit a signed copy to the VMA Athletic Office once a school year if they are participating in a high school sport. The signed policies will be kept in the student’s athletic file along with his/her current KHSAA eligibility/physical form.
